Important Notice Regarding Permanent Closure of Total Urgent Care Clinics

Both Total Urgent Care locations in Cut Off and Vidalia, Louisiana, officially closed on March 1, 2025. We want to thank our patients and communities for their trust and support over the years—it’s been our privilege to care for you.

As a follow-up to the closure, we would like to extend the opportunity for you to obtain a copy of your medical records. Requests for records must be submitted in writing. Please direct your requests to Datavant (CIOX) Requests using the following contact information:

Please be advised that the deadline for submitting medical record requests is November 1, 2025. After this date, we will no longer be able to process requests through this office.

When submitting your medical record request, please include:

  • Your full name (and any prior names used)
  • Date of birth
  • Contact information (phone and mailing address)
  • The specific records you are requesting (e.g., entire chart, lab results, imaging reports)
  • The name and address of the recipient if you would like records sent directly to another provider
  • Your signature and date of request
